Manually Create appointment Add ons not working

      Hello,

      I am trying to manually create an appointment from store front through menu: Appointments->Create Appointment.
      But the addon fields (red circle in screenshot) are not working. Checkboxes cannot be even clicked. Dropdown value can be selected but it wont update price and it wont be shown in the created order.
      In WordPress dashboard the manual creation of appointments works perfectly. The problem exists only in Store Front.
